Enrollment

Destiny High School is currently accepting applications for the 2009 - 2010 school year. You will find the admission process below in a few simple steps: (You must have adobe reader to view downloads. Get it here )

Step I:         Submit application (click here to download)

• - Report card or transcript for transfer students
• - 2 recommendations from teachers /or administrator
• - 1 recommendation from clergy or church youth leader (click here to download)

Step II:         Complete and submit Choice Application (click here to download)

• - Include proof of address (utility bill, etc.)
• - 2008 income tax documents
• - Income tax documents for all working people in household

Step III:       Private Pay Families Schedule appointment with Destiny Financial office (414-354-4430)

Step IV:       Other Form

• - Student Immunization Records

Step V:        Schedule interview for parent and child

Step VI:       Schedule Entrance Exam

Step VII:      Sign & Return Student Enrollment form received with acceptance letter.
